How do I change the name on my holding?

If you have an issuer sponsored holding, you can change your registered name by completing the Name Correction Request & Indemnity form. You can obtain the form by:

• Using the Investor Portal: if you have already registered, enter your username and password and click "log in". If you have not yet registered, click "register" and follow the prompts. Once you have logged in, click on "Forms" on the left and then click on "Name Correction Request & Indemnity" to download the form. 
• Downloading the form (see below). 
• Contacting our office: contact us by email or by phone to request a form.

Please ensure you provide your Securityholder Reference Number, all signatures are witnessed, and supporting documentation is provided as specified on the form.  

Once the form has be en completed and signed, please return the form to our office via email or post. 

Please note if you want to change the name on the holding to a completely different name, a Standard Transfer Form will need to be completed instead of a Name Correction Request & Indemnity form. 

If you have a CHESS-sponsored holding (also known as a broker-sponsored holding) please contact your broker to change the name on your holding.
